Serum parathyroid hormone levels and renal handling of phosphorus in patients with chronic renal disease.

چکیده
In eight patients with advanced renal insufficiency (inulin clearance 1.4-9.1 ml/ min), concentrations of serum calcium (S[CaJ) and phosphorus (S[PJ) were maintained normal (S[Ca] > 9.0 mg/100 ml, (S[P] < 3.5 mg/lOO ml) for at least 20 consecutive days with phosphate binding antacids and oral calcium carbonate. The initial serum levels of immunoreactive parathyroid hormone (S-PTH) were elevated in three (426-9230 pg/ml), normal in four (one after subtotal parathyroidectomy), and not available in one. The initial fractional excretion of filtered phosphorus ( Cp ) was high in all and ranged CIN from 0.45-1.05. Following sustained normo-calcemia and normo-phosphatemia, S-PTH was reduced below control levels in all patients; being normal in six and elevated in two. Cp decreased below CrN THE DECREASED fractional tubular reabsorption of phosphorus in patients with chronic renal disease has been attributed to concurring secondary hyperparathyroidism (1-3). However, the persistence of high fractional excretion of phosphorus after parathyroidectomy (4-6), even with demonstrable normal serum level of immunoassayable parathyroid hormone (6), supports the notion that additional factors other than secondary hyperparathyroidism may play an important role in the regulation of tubular reabsorption of phosphorus in patients with advanced chronic renal insufficiency. Intensification of Anemia by Secondary Hyperparathyroidism in Hemodialysis Patients
The excessive amounts of parathyroid hormone in secondary hyperparathyroidism (SHPTH) is suggested to interfere with normal erythropoiesis. In SHPTH, during chronic renal failure, due to the impairment of erythropoietin synthesis, this effect is more pronounced. In the present study the role of secondary hyperparathyroidism in the severity of anemia was evaluated in hemodialysis patients (n=3...
متن کاملFactors Associated with Uremic Pruritus in Patients Undergoing Hemodialysis: A report from Arak Valiasr Hospital
Background: Pruritus is one of the most disabling problems in patients with chronic renal failure. It is a distressing symptom with a negative impact on quality of life and is also very frustrating for both patients and their physicians. The pathophysiological mechanisms of pruritus are mainly unknown. The aim of this study was to identify factors associated with pruritus in these patients. Met...
متن کاملThe need for better control of secondary hyperparathyroidism.
Secondary hyperparathyroidism (SHPT), a frequent complication of chronic kidney disease, develops in response to an imbalance in the serum levels of calcium, phosphorus and vitamin D as a result of altered metabolism. Raised serum levels of parathyroid hormone (PTH) and calcium-phosphorus product have a major effect on morbidity and mortality in dialysis patients. The new Kidney Disease Outcome...
متن کاملThe relationship between hemoglobin level with PTH level and dialysis adequacy in chronic hemodialysis patients
Background: The role of high levels of parathyroid hormone in the development of anemia either directly or by mediating mechanisms is unclear, and there is a controversy about the pathophysiological role of parathyroid hormone (PTH) in the development and intensification of anemia associated with renal disease. The aim of this study was to investigate the relationship between anemia and parathy...
متن کاملThe Pathophysiology of Secondary Hyperparathyroidism and the Consequences of Uncontrolled Mineral Metabolism in Chronic Kidney Disease: The Role of COSMOS
The development of secondary hyperparathyroidism (SHPT) is a common complication of chronic kidney disease. SHPT develops as a consequence of mineral metabolism disturbances and is characterized by elevated serum parathyroid hormone (PTH) and parathyroid hyperplasia. Evidence suggests that SHPT contributes to the development of vascular calcification and cardiovascular disease, as well as to th...
